 It is with a heavy heart that we announce Primary Care will officially be retiring on April 30th. It has truly been a privilege to serve you and walk alongside you on your health journey.

We want to provide accessible healthcare regardless of your financial position, which is why we accept ALL insurances and provide a sliding fee scale for those without. If you are paying out of pocket and are in need of financial assistance to cover your health care, please ask our front desk for a sliding scale application. You'll also be asked to include either your most recent paystub or last year's taxes when you submit your application.
