Our Primary Care Providers are here to assist in building a healthier you. From sports physicals to referrals for injuries, annual exams, managing ongoing health issues or when facing unexpected illness; we treat it all.

New Patients:  

You can either download the packet (below) or come in to fill out a new primary care patient packet. 

Upon return of the packet, we will need ID and insurance information (if you have it). Our staff will review the packet and call you to schedule an appointment.  

Established Patients:

  You can now schedule a telehealth visit, inquire about your health, and request a prescription refill using your Patient Portal!

For all follow up visits, please use the patient portal or call the primary care number to schedule an in-person or telehealth appointment.
