We know that urgent care is never on your itinerary for the day, but we are here for you when need us most. 

 

 Appointments are walk in scheduling, with exception to telehealth self-scheduling. Please come in at your earliest convenience to reserve a time slot. This allows you to go about your day if the wait is long.


We will need your ID, insurance Information (if you have coverage), and form of payment at time of check in.

If you are a new Urgent Care Patient, please download and fill out the attached file below. You can either email or come by and see us, while dropping it off in person. If you are unable to do so, a copy will be provided at the clinic.
